This program is administered by the Department of Human Services and provides homemaking services, non-medical
personal care, and instructional care to individuals functionally unable to perform life-essential tasks of daily living.
The objective of the program is to maintain high-risk individuals “in place” – maximizing independence by preventing or
slowing the disabling process.
Consumer must be determined eligible for services by a formal need assessment conducted in the home by one of our
coordinators.
