Landmark Home Health Care is a non-profit, Medicare certified agency dedicated to helping patients achieve the highest
possible quality of life through a prompt response to patient needs. By assigning a Nurse Care Manager to coordinate
service to each patient, we provide an accurate, continuous course of treatment that is closely monitored.
All services are provided in accordance with a physician signed plan of care to assure that a patient’s return to optimum
health is maximized.

A team of skilled and experienced registered nurses (RN’s) provide direct care, instruction and referrals to other needed
services under the supervision of your physician.

voice exercises, language processing and memory strategies are provided.
Occupational Therapy
Licensed occupational therapists and assistants provide Activities of Daily Living Training, homemaking skills, functional
ambulation, range of motion exercises and transfer training.
Medical Social Services
Social workers evaluate the patient’s personal and financial needs, and link patients with sources of assistance.. They also
provide counseling to patients and their family caregivers.
